അർഹതാ മാനദണ്ഡം – ഇൻഷുറൻസ് പരിരക്ഷയില്ലാത്ത ഒരു വർഷമെങ്കിലും തുടർച്ചയായി അംശാദായം അടച്ചിട്ടുളള അംഗങ്ങൾക്ക് അഞ്ച് ദിവസമോ അതിൽ കൂടുതലോ ആശുപത്രിയിൽ കിടത്തി ചികിത്സയ്ക്ക് വിധേയരാകുമ്പോൾ

അപേക്ഷിക്കേണ്ട വിധം – ബന്ധപ്പെട്ട ജില്ലാ ക്ഷേമനിധി ഓഫീസിൽ നിന്ന് നിർദ്ദിഷ്ട ഫോം വാങ്ങി പൂരിപ്പിച്ച് ഡിസ്ചാർജ്ജ് സർട്ടിഫിക്കറ്റ്, മെഡിക്കൽ സർട്ടിഫിക്കറ്റ്, ചികിത്സാ രേഖകൾ എന്നിവ സഹിതം ബന്ധപ്പെട്ട ജില്ലാ ക്ഷേമനിധി ഓഫീസർക്ക് സമർപ്പിക്കേണ്ടതാണ്.
